Abstract

CareMonitor is a patient-driven self-monitoring tool available for mobile devices that empowers individuals to actively manage their chronic disease. The app enables patients to record key health metrics such as blood pressure, weight, and heart rate while providing access to valuable educational resources that help them better understand and manage their condition. This user-friendly application is designed to support patient education and foster greater engagement in the self-management process.

In addition to the patient interface, CareMonitor includes a fully integrated dashboard for general practices and a web-based dashboard accessible by clinicians in the Murrumbidgee Local Health District (MLHD). The clinician dashboard allows the patient's care team to track self-collected metrics in real time and engage directly with patients. If a patient’s self-recorded readings fall outside recommended values, clinicians are alerted instantly, enabling timely intervention and advice. The app supports proactive care by allowing clinicians to send health risk assessments, questionnaires, and tailored advice to patients, either on demand or at pre-determined intervals.

Through this real-time monitoring and communication, CareMonitor empowers patients to better understand their health, take responsibility for their self-care, and know when to seek help. Over 145 patients are currently enrolled across 16 general practices and 71 clinicians are onboarded. Feedback from a research project conducted in collaboration with Charles Sturt University (CSU) indicates that patients found the app incredibly beneficial. They reported enhanced confidence in managing their chronic conditions, with many citing an improved understanding of when to reach out for professional care.

Looking ahead, CareMonitor represents a transformative shift towards patient-centred care, where education, self-monitoring, and clinician collaboration work together to optimise health outcomes and improve overall healthcare efficiency. By giving patients, the tools and support they need, CareMonitor is laying the foundation for a more empowered and proactive approach to chronic disease management.

Biography

Elise and Danniell work together under the Collaborative Commissioning initiative Living Well, Your Way. Together with Danniell's expertise in Digital Health and Elise's clinical experience as a Registered Nurse, they joined forces to support patients with Chronic Disease through new innovative technology.
